New Client Paperwork

All clients must complete our intake paperwork before they are able to be seen. These forms may be completed either by manually and submitted via email or electronically using our secure client portal. Paperwork must be submitted no later than 48 hours before your appointment time or your appointment will be automatically canceled.

Please note:

  • Couples will each need to complete an individual copy of the paperwork.
  • Clients age of 14+, the client & the parent must sign where each signature is required.
  • For clients age of 14+, it is strongly recommended you fill out an Authorization to Release Information Form for third parties (e.g., a parent, spouse, or healthcare provider) you want to include in your care, including making appointments and discussing billing.
  • Do not hit Save and Complete until the entirety of your document is completed. Once selected you will not be able to make changes on your document.
